History
Crestview was built in 1974 and taken over by Paula and Azzy Reckess in 1994.
Paula and Azzy invested in the property, increased staffing levels, and created a family-focused environment. They have continuously maintained Crestview as a facility "In Good Standing" with the NYS Department of Health since taking over.
Crestview has been the recipient of various letters of support and commendation from regulatory agencies, including both the NYS Dept of Health (DOH) and the Veterans Affairs Administration (VA).
Crestview is managed by PAZ Management, Inc, of Poughkeepsie, NY (also owned and operated by the Reckess family).
Crestview maintains two “sister facilities”, including:
Dutchess Care ALP, an assisted living community in Poughkeepsie, NY (Dutchess County) providing Medicaid ALP services to adults with chronic mental health illnesses. Dutchess Care also provides services to adults with Traumatic Brain Injuries (TBI).
Avalon Assisted Living & Social Day Program, an assisted living community in Wappingers Falls, NY (Dutchess County) providing Medicaid ALP, private pay Adult Home and Social Day Program services to the geriatric community.

Crestview is located one block from Route 9A in the Hamlet of Hawthorne, making it easily accessible by car Route 9A, Saw Mill River Parkway, Sprain Brook Parkway) and by public transit (Metro North Train to Hawthorne, and Bee Line Bus Service).
The backyard of Crestview is a residential community, while the front door provides access to Westchester's commercial amenities, including diners, banks, and post offices.
Additionally, Crestview is close to many of the region's best medical providers: We are a 4 minute drive to Westchester Medical Center, an 8 minute drive to Phelps Memorial Hospital, and a short distance from multiple other community medical and mental health providers.
